NOTICE TO CLIENT (Required by California Business & Professions Code §2053.6 – SB-577)

I, Kayla Terzi, am a Certified Reiki Master Practitioner providing complementary and alternative health care services. I am not a licensed physician, surgeon, psychologist, psychotherapist, or other licensed health care professional in the State of California. Reiki is a complementary, holistic energy practice intended to support relaxation, stress reduction, and overall well-being. Reiki is not a substitute for medical diagnosis, treatment, or licensed mental health care. If you have a medical or psychological condition, you are encouraged to seek care from an appropriately licensed health care professional.

DESCRIPTION OF SERVICES

Reiki is a Japanese energy healing technique that involves gentle placement of hands on or above the body to support relaxation and energetic balance. Reiki sessions are intended to promote relaxation and well-being. Individual experiences vary, and additional complementary healing modalities will only be incorporated into your session with your consent below.

CLIENT ACKNOWLEDGMENTS

By signing below, I acknowledge that:

  1. I understand that Reiki is a complementary modality and not licensed medical care.

  2. I understand that no diagnosis, prescription, or medical treatment is being provided.

  3. I understand that Reiki does not replace medical or psychological treatment.

  4. I am responsible for consulting a licensed medical professional regarding any medical condition.

  5. I understand results are not guaranteed.

  6. I voluntarily consent to receive Reiki services and complementary holistic modalities from Kayla Terzi.

ASSUMPTION OF RISK & RELEASE OF LIABILITY

By signing below, you acknowledge that…

  • I understand that Reiki is generally considered safe and non-invasive.

  • I voluntarily assume any risks associated with participating in sessions.

  • I agree to release and hold harmless Kayla Terzi from any liability, claims, or damages arising from participation in Reiki sessions, except in cases of gross negligence or intentional misconduct.
